OCCUPANCY PERMITS

Form 36-36 Inspection Statements(if not applicable, indicate NA)
Form 36-37 Confirmation of Called Inspection Records(if not applicable, indicate NA)

Temporary or Mobile Unit Facility Occupancy Permits (required annually)

STATEMENT OF COMPLETION
(For Work Not Impacting Occupancy)
