Current food health permits expire on April 30, 2025. Use this form to renew your health permit online. 

You will need your Health Permit Fee and Site Number off the invoice that you received in the mail from the Department. The permit fees are based upon the number of seats or checkouts and the risk classification of the food establishment.  If you did not receive your renewal invoice, please contact the Division of Environmental Health at 815-334-4585 to obtain your permit renewal fee and site number. 

You will receive your new health permit, valid from May 1, 2025 until April 30, 2026 in the mail.

PAYMENT INSTRUCTIONS

The McHenry County Department of Health now accepts credit and debit card payments through GovPay.Net.  You will need the following information to submit your payment:

   1. The permit fee (found on your health permit renewal invoice).
