• Onsite Wastewater Treatment System Site Evaluation Application -- Review of Soil Borings for Septic Suitability 
  Written evaluation of the suitability of a site to install a compliant onsite wastewater treatment system to serve a new residence or structure.

• Sale of Property Evaluation Form - Septic System 
  An evaluation of the current operation of an onsite wastewater treatment system typically done for real estate transactions and/or refinancing of mortgages.

• Septage Management Site Permit Application 
  Required to store, dispose or use septage at any site in McHenry County except an Illinois Environmental Protection Agency permitted sewage treatment plant.

• Septage Management Site Evaluation Application
  An evaluation of the feasibility of a site to meet Ordinance requirements for use as a septage management site.

• Private Sewage Disposal System Pumping Contractor & Portable Toilet Technician Permit Application 
  Required to collect, store, transport, dispose or use septage, portable toilet waste or domestic sewage from a septic tank, holding tank, privy vault or sanitary dump station.

  An evaluation of the current operation of a water well typically done for real estate transactions and/or refinancing of mortgages.  Includes analysis of water sample for coliform bacteria and nitrates.

• IDPH Non-Community Public Water System Application for Permit to Construct, Alter, or Extend a Drinking Water System 
  Required by Illinois Department of Public Health to construct or modify a non-community public water supply.

• Illinois Department of Public Health Application to Construct, Modify, or Abandon Well 
  Required by Illinois Department of Public Health in addition to the county permit to construct, modify or abandon a water well.

• Illinois Department of Public Health Application to Construct, Modify, or Seal a Closed Loop Well System 

• Chain of Custody Form
  Must be completed and submitted to the laboratory when a water sample is submitted for analysis.

Open Burning of Landscape Waste Applications

  Required for an outdoor ceremonial fire as defined in Article II of the McHenry County Public Health Ordinance.
