Requesting a death certificate
You may obtain a death certificate in one of 3 methods:
1. Order Online – please click on logo below.

Include the following items:
- A completed Death Certificate Request Form
- A copy of your government issued photo identification
- A check or money order payable to the McHenry County Clerk for a $16 search fee for each death record - this fee will be returned if no record is found. Additional copies of the same record issued at the same time will be charged a fee of $8.00 each.
- A self-addressed stamped envelope
- Funeral Homes requesting need to provide: A copy of the Funeral Directors License OR a letter on their letterhead including a copy of a government issued ID of the person requesting the record. Also a document stating the legal need or financial interest is necessary.
- If you are not a relative of the deceased person, a letter or document from the office or agency needing the Death Certificate must accompany the request

The McHenry County Clerk's Office cannot process orders over the phone: however, for your convenience, you can submit an online request through a third-party service named VitalChek.
