Acceptable forms of Identification

To obtain certain vital records, you are required to submit one or more copies of ID. All forms must be current and valid. 

One form of ID is required if submitting one of the following IDs:

• Current driver’s license or state identification card (issued within the United States)
• U.S. Passport
• U.S. Naturalization Certificate
• U.S.  Military Identification Card
• U.S. Selective Service Card
• U.S. Immigration Card (resident alien)
• Consulate Identification Card (matricular/tarjeta)
• Foreign Passport (with current U.S.A. issued visa)
