The Request for Change of Address form is for any property owner who needs to change the mailing address on his or her parcel(s).  Only the legal owner, trustee or person holding power of attorney from the legal owner or trustee can make changes to the mailing address.  (Property Tax Code 35ILCS-200/20-20)

If the parcel(s) is held in a bank trust or a business name please include proof, along with the form, which states that you are authorized to change the tax mailing address.  A copy of a permanent change of address filed with the US Post Office will suffice as proof.
