Family Case ManagementCase Management provides support for infants and children with high-risk medical conditions and referrals to community and social services.

Who can receive  services and what does it cost?
If you have a child under the age of 24 months with a high-risk medical condition you may be eligible. There is no cost to clients for services provided.

  • Early Intervention
  • Well child exams and immunizations
  • All Kids (Health Insurance)
  • Domestic Violence

Our dedicated Public Health Nurses can see you in the office and in your home. Home visiting services provide the opportunity to evaluate your baby in their own environment and learn more about household safety tips.
