The McHenry County Clerk's office keeps official records of deaths that occur in McHenry County.

Requesting a death certificate

You may obtain a death certificate in one of 3 methods:

NOTE:   DEATH RECORDS REQUESTED FROM FUNERAL HOMES WILL BE ISSUED THE SHORT FORM OF THE DEATH RECORD WHICH IS CERTIFIED AND CAN BE USED FOR LEGAL PURPOSES.    IF A FAMILY REQUIRES THE LONG FORM OF THE DEATH RECORD THE FUNERAL HOME CAN HAVE THE IMMEDIATE FAMILY MEMBER SIGN THE COMPLETED REQUEST FORM AND ATTACH A COPY OF THAT FAMILY MEMBER'S ID AND INDICATE WHERE THEY WANT THE CERTIFIED COPIES SENT TO. 

2.   Write us –

 Include the following items:

  • A completed Death Certificate Request Form*
  • A photocopy of your photo identification
  • A check or money order payable to the McHenry County Clerk for a $16 search fee for each death record - this fee is retained even if no record is found. Additional copies of the same record issued at the same time will be charged a fee of $8.00 each.
  • A self-addressed stamped envelope
